Form Explanations
Enrollment Form: This form must be completed for each camper enrolling in camp. Download
Payment Plan: This form must be completed for each family enrolling in camp. Multiple children can be included on this form. Download
**This form only needs to be completed if you are NOT enrolling online.
Camp Arye Payment Plan Form: This form must be completed by all campers who receive 3rd party funding. This form must be turned in at the same time as the Enrollment Form. If you enrolled online, you will not need to complete this form. Download
Camper Health History Form: This form must be completed before your child can attend camps Ora, Shemesh, Chaverim, Hoover or Arye. This form does not need to be filled out by a doctor. Download
Membership Application: This application must be completed if you are NOT currently a JCC member. All campers must be members in order to attend camp. Download
Scholarship Application: Completed scholarship applications must be submitted to Ralph Rothschild. Scholarships are competitive and are need based. Campers who receive third party funding are not eligible for scholarship. Download Letter • Download Application
For Camp Yeladim (College Avenue):
Child Enrollment and Health Information Form: this form is required for all campers NOT currently enrolled in any JCC preschool program. Enrollment in a preschool program includes all 3 preschool sites. Download
Child Medical Statement: This form is required for all Yeladim campers that are NOT currently enrolled in a JCC preschool program. This form must be completed by a physician. Download
Swim Permission Form: This form must be completed in order for your camper to swim at Camp Yeladim (College Avenue).
Request for Administration of Medication: This form must be completed for any campers who require medicine during the camp day. Medication includes any topical product or lotion which includes sunscreen. If you want camp staff to be able to help apply sunscreen to your camper this form must be completed.
Child Medical/Physical Care Plan: This form must be completed for any camper who requires ongoing medical treatments such as for asthma, tube feedings or epi pens.
For Camp Yeladim and Shemesh (New Albany) only:
